PopHIVE partners with Metopio to deliver actionable health data

The Yale School of Public Health’s Population Health Information and Visualization Exchange (PopHIVE) has partnered with Metopio, a national community health data automation platform, to make health data easier to use where it matters most: in decisions that affect patients, families, and local communities.

PopHIVE is a public-good platform that provides timely, trustworthy, de-identified population health data. It brings together information from clinical care, public health systems, and other community sources to help people understand what’s happening in their communities and respond faster to health concerns.

Metopio works directly with local health departments, hospitals, and community organizations across the country. Its platform helps leaders move from static reports and spreadsheets to clear, actionable insights. It’s purpose-built for collaboration, efficiency, and impact, empowering organizations to navigate community health challenges and drive meaningful change.

Through the partnership, PopHIVE’s dashboards will be integrated into the Metopio platform, allowing local health leaders to merge high-quality national data with their own community context to make informed decisions that improve health outcomes.

“This partnership is about making modern public health data usable where it matters most — at the local level,” said Dr. Anne Zink, MD, a senior fellow at Yale School of Public Health and co-director of PopHIVE. “Metopio not only helps bring PopHIVE’s indicators into community workflows; they’re also helping us hear what communities need for population health data to drive meaningful data into information that is actionable.”

Some of the immediate benefits of the partnership include:

  • Bringing trusted data into everyday decision-making. PopHIVE’s multi-source health indicators will be embedded into tools that local public health and health care teams already use.
  • Turning data into action. Pairing national trends with localized insights will help community leaders better identify risks, close gaps, and respond faster to emerging concerns.
  • Strengthening the use of community context data. Metopio will help ensure that measures such as housing, income, education, and access to care are presented in ways that are practical and actionable.
  • Presenting community-friendly data. Both teams will focus on building tools that are clear, usable, and centered on improving lives.

Initial areas of focus will include respiratory diseases, childhood immunizations, chronic diseases, and injury and overdose.
