Patient & Caregiver Resources

Navigating the complexities of aging can be difficult, especially as it applies to health care. Yale Geriatrics has developed a suite of evidence-based resources for patients and caregivers covering topics such as long-term care planning, dementia and Alzheimer’s disease, caregiver support, and managing many chronic conditions.

These resources are funded by an award from the Health Resources and Services Administration (HRSA) as part of its Geriatric Workforce Enhancement Program (COACH 4M). The PATH and STAMP Guides were supported by the Humana Foundation and the National Institutes of Health/National Institute on Aging (award P30 AG021342).
