  • 00:00Hi everyone. Welcome to the
  • 00:02Yale Hospice and Palliative Medicine
  • 00:04Fellowship Program.
  • 00:05I'm Doctor. Alex Choi, one
  • 00:07of our program faculty and
  • 00:09twenty twenty one graduate of
  • 00:10the program.
  • 00:12We're excited to meet you,
  • 00:14and we hope this video
  • 00:15will give you a sense
  • 00:16of our fellowship experience.
  • 00:18Let's start with a day
  • 00:19in the life of a
  • 00:20palliative care fellow on our
  • 00:21inpatient consult service.
  • 00:24Our fellows begin their day
  • 00:25in the fellows workroom, which
  • 00:27they share with the Hmong
  • 00:28fellows.
  • 00:29Here, they review charts and
  • 00:31prepare for the day.
  • 00:32It's important to have a
  • 00:34dedicated space to focus.
  • 00:36Then we have our daily
  • 00:37interdisciplinary
  • 00:38morning huddle to run the
  • 00:39list and distribute consults.
  • 00:41And yes, we're always as
  • 00:43happy at work. Our inpatient
  • 00:45consults come from all areas
  • 00:46of the hospital. Let's stop
  • 00:47here at the fountain on
  • 00:48the first floor where we
  • 00:50can see the four pavilions
  • 00:51of the hospital.
  • 00:52West Pavilion has the CT,
  • 00:54ICU, and Transplant unit. It
  • 00:56also houses pediatrics.
  • 00:58South Pavilion has the ED
  • 01:00and the Neurology, Cardiology, and
  • 01:02Surgical units.
  • 01:04East Pavilion is home to
  • 01:05the med surg beds.
  • 01:07And North Pavilion houses the
  • 01:09medical ICU and several oncology
  • 01:11units including separate floors for
  • 01:13Hematology,
  • 01:14Oncology, and Gai Nong.
  • 01:16After seeing patients, Fellows round
  • 01:18with their attending.
  • 01:19Our faculty place a strong
  • 01:21emphasis on teaching, using every
  • 01:23opportunity to support Fellows in
  • 01:25reaching their learning goals and
  • 01:26advancing toward independent
  • 01:28expert practice.
  • 01:30It's time for lunch! The
  • 01:32cafeteria is fine, but you
  • 01:33can't miss out on the
  • 01:34food carts in front of
  • 01:35the medical school. There's halal,
  • 01:38arepas,
  • 01:39banh mi, bibimbap,
  • 01:40salads,
  • 01:41and lots more to choose
  • 01:42from.
  • 01:46In the afternoon, Fellows continue
  • 01:47bedside rounds unless they are
  • 01:49scheduled for their weekly half
  • 01:51day clinic session.
  • 01:52Here's a look at our
  • 01:53palliative care clinic on the
  • 01:55fourth floor of North Pavilion.
  • 01:57Fellows are paired with a
  • 01:59different attending for each half
  • 02:00of the year in our
  • 02:01Cancer Centre Palliative Care Clinic.
  • 02:03Beyond this, they gain broad
  • 02:05exposure to ambulatory palliative care
  • 02:07for patients with non cancer
  • 02:09diagnoses.
  • 02:10At the VA, for example,
  • 02:12fellows join palliative care clinicians
  • 02:14embedded within ALS, pulmonary, and
  • 02:17hepatology teams.
  • 02:19At our Saint Raphael's campus,
  • 02:21they experience our embedded heart
  • 02:22failure clinic.
  • 02:24And that's it for the
  • 02:25day.
  • 02:26Wait, is that
  • 02:28sunlight?
  • 02:29During your time at Yale,
  • 02:30you'll appreciate the many unique
  • 02:32things that are available at
  • 02:33our institution.
  • 02:35One example is the Oncology
  • 02:36Extended Care Clinic or ECC,
  • 02:39a dedicated space within the
  • 02:41hospital that allows our oncology
  • 02:43patients to bypass TED
  • 02:45and receive urgent care.
  • 02:47During the height of the
  • 02:48COVID-nineteen
  • 02:49pandemic,
  • 02:50this space was converted to
  • 02:51the supportive care unit, where
  • 02:53our palliative care team took
  • 02:54care of patients who are
  • 02:55comfort measures only.
  • 02:58On the seventh floor is
  • 02:59the healing garden, another unique
  • 03:00place available to patients, families,
  • 03:03and staff.
  • 03:08Though our fellows spend a
  • 03:09large part of the year
  • 03:10at Yale New Haven Hospital,
  • 03:11they also have a diverse
  • 03:13exposure to several hospice and
  • 03:14palliative care settings.
  • 03:16They spend three weeks with
  • 03:17the palliative care team at
  • 03:19Westhaven VA,
  • 03:20and three weeks with Connecticut
  • 03:22Hospice, the first hospice in
  • 03:23the US.
  • 03:25Fellows also rotate through Middlesex
  • 03:27Hospice for four weeks.
  • 03:28And later in the year,
  • 03:29they spend two weeks learning
  • 03:31about the role of hospice
  • 03:32medical director
  • 03:33with Doctor. Shaben, a regional
  • 03:35medical director for VITAS.
  • 03:37Fellows also spend two weeks
  • 03:39with the Sunflower Palliative Care
  • 03:40team at Connecticut Children's Hospital.
  • 03:45Fellows appreciate their dedicated scholarly
  • 03:48blocks, which provide them time
  • 03:49and support to pursue quality
  • 03:51improvement
  • 03:52and other scholarly projects.
  • 03:54These initiatives have made a
  • 03:56lasting impact on our program.
  • 03:58For example, streamlining our inpatient
  • 04:00sign up process,
  • 04:02enhancing inpatient and outpatient note
  • 04:04templates, and much more.
  • 04:06We are proud of our
  • 04:07mentorship program that pairs each
  • 04:09fellow with a faculty member
  • 04:11with similar professional interests.
  • 04:13Mentors serve as a resource
  • 04:15throughout the fellowship, from everyday
  • 04:16questions and concerns,
  • 04:18to guidance on scholarly projects,
  • 04:20to support during the job
  • 04:22search.
  • 04:23Outside of work, there are
  • 04:24plenty of fun things to
  • 04:25do. New Haven is centrally
  • 04:27located between Boston and New
  • 04:28York City,
  • 04:29making it convenient to access
  • 04:31either of them on the
  • 04:32weekends.
  • 04:33But you don't have to
  • 04:34go that far. Art museums,
  • 04:36parks, beaches,
  • 04:38and of course the famous
  • 04:39Connecticut Apetes.
  • 04:42New Haven has a diverse
  • 04:43culinary scene with a variety
  • 04:45of authentic cuisines from around
  • 04:47the world.
  • 04:58The Yale University campus is
  • 05:00also located a few blocks
  • 05:01from the hospital.
  • 05:03New Haven is fun to
  • 05:04explore at night too, with
  • 05:05options for restaurants, bars, music,
  • 05:08and theater venues.
  • 05:10We thank the various organizations
  • 05:12who are dedicated to enriching
  • 05:13the culture of downtown New
  • 05:15Haven. For those who are
  • 05:16looking to get out of
  • 05:17the city, the Connecticut shoreline
  • 05:19is home to beautiful views
  • 05:20and tasty lobster rolls.
  • 05:22Families also do day trips
  • 05:24to the beautiful little towns
  • 05:25in the northwest corner of
  • 05:26the state.
  • 05:28Our program provides you with
  • 05:30rigorous clinical training along with
  • 05:32resources and support to help
  • 05:34you thrive,
  • 05:35so you leave prepared for
  • 05:36any career path you might
  • 05:37choose.
