      <title>Contextual factors influencing bubble continuous positive airway pressure implementation for paediatric respiratory distress in low-income and middle-income countries: a realist review</title>
      <description>This realist review aimed to explore the factors influencing the implementation of bubble continuous positive airway pressure (bCPAP) therapy for children aged 1–59 months in low- and middle-income countries (LMICs), and to develop a theory explaining how these factors affect outcomes. Through analysis of 38 peer-reviewed and 2 grey literature sources, the researchers identified 18 context–mechanism–outcome configurations (CMOCs), which were synthesized into a final program theory. This theory operates at five levels: the bCPAP device, local partnerships, clinical teams, caregivers, and institutional practices, and offers insights into how bCPAP effectiveness depends on contextual factors. Wegovy and Zepbound show significant weight loss results but are not covered by Medicare and are expensive. Their effectiveness and safety in older adults are uncertain since FDA studies included few participants over 64. Concerns about muscle mass loss and different treatment goals for older adults make it essential to ensure these drugs are safe and beneficial.
More research is needed to confirm the safety and efficacy of these medications for older adults. Until then, lifestyle changes like reducing late-night snacking might be a safer approach.</description>
